How to Apply — Hospice and Palliative Medicine Fellowship at St. Elizabeth Boardman Hospital
Candidate Eligibility
The Hospice and Palliative Medicine Fellowship Program at Mercy Health – St. Elizabeth Boardman Hospital will accept two fellows for PGY-4 positions each year. Basic requirements for consideration of applications:
- United States citizen, permanent resident or able to obtain J-1 or H-1B Visa.
- Graduate of an accredited residency program.
- Successful completion of Step 3 of USMLE/COMLEX.
- International medical school graduates must have a valid ECFMG certification or a full and unrestricted license to practice medicine in the U.S.
ERAS and Match Program Participation
Applications are accepted only through the Electronic Residency Applications System (ERAS). Our program participates in the National Resident Matching Program (NRMP).ACGME Program Number: 5403812002
Application Requirements
Completed ERAS application, including:
- An official Dean’s Letter or equivalent from medical school
- Two or more professional reference letters
- Personal statement
- Curriculum vitae (CV)
- Medical school transcript
- USMLE/COMLEX scores
