Mercy Health Physicians is dedicated to ensuring consistent and quality care to our patients. By addressing the management of conditions such as blood pressure, diabetes and depression early on, we hope to prevent or mitigate risk factors of more severe disease later on.
Cancer screenings for breast, colon and lung are also a priority for our Ministry, with a focus on reducing cancer through prevention and early detection.
One of the benchmarks we hold ourselves to is our Medical Group Clinical Quality metric, which outlines our goals and commitments for addressing common medical concerns in our community.
Social Determinants of Health
It is estimated that 80% of health outcomes are determined by social and economic factors. Differences that exist in social determinants of health (SDoH) can lead to health care disparities and inequities. Our Mission emphasizes caring for people who may struggle financially or lack adequate health care coverage. This is often reflected through a patient’s unmet social needs, such as access to healthy food or reliable transportation.
Our practices screened 80.5% of the adult patients seen in our primary care practices for food and financial insecurity in 2023. The efforts of our primary care providers and their care teams provided insights about our patients to help provide better outcomes for them and the communities we serve.
In 2024, we plan move beyond just screening for these factors to actionable plans by providing information on community-based resources to meet their needs.
