Mercy Medical Associates First in Region to Earn National Certification

Paducah, KY (July 20, 2016) - Mercy Medical Associates is pleased to announce its nine primary care practices and its pediatric practice have achieved Level 3 Patient-Centered Medical Home (PCMH) status, the highest, most stringent qualification standard for a PCMH. 
This national recognition comes from the National Committee for Quality Assurance (NCQA). 

The PCMH program is an innovative program for improving primary care. It helps medical practices organize care around patients, work in teams and coordinate and track patient care over time to ensure patients benefit from the highest quality care delivered at the best possible cost.

In a PCMH:
Each patient has an ongoing relationship with a personal physician who provides the patient with continuous and comprehensive care.
The personal physician leads a team that includes registrar and a medical assistant. 
The entire practice works as a team along with a nurse care coordinator who partners with patients to help them reach their health goals. 
The PCMH coordinates and integrates the patient’s care across all elements of the healthcare system and the patient’s community.
Patients can access a behavioral health specialist and a social worker. 
Quality and safety are hallmarks of the medical home.
Enhanced access to care is available through open scheduling, expanded hours and other innovative options for communication between patients, their personal physician and practice staff.
