St. Rita’s receives recognition from the National Committee for Quality Assurance

(Lima, May 2, 2016) Several family medicine practices of St. Ritas Professional Services have been awarded Level 3 Recognition by the National Committee for Quality Assurance (NCQA) Patient-Centered Medical Home (PCMH) program. The PCMH program measures certain aspects of care including: access and communication; patient tracking and registry functions; care management; patient self-management support; electronic prescribing; test tracking; referral tracking; performance reporting and improvement; and advanced electronic communications.

This recognition was a renewal for the following St. Ritas Professional Services practices:Family Medicine Associates Klass Family Medicine Martin & Martz Family Medicine Primary Care AssociatesSpencerville Family Physicians
This was a first-time recognition for the following practices:
Delphos Medical Associates St. Rita's Family Medicine at Columbus Grove

PCMH focuses on strengthening the relationship between the patient and the physician while providing typical family medicine services. With the medical home model, patients are active participants in their care and the primary care physician serves as the home, or the office where patients go for the majority of their medical needs. The medical home model focuses on a team approach. Primary care physicians, specialists and the patient all communicate and work towards the same health care goal, through patient counseling, discussion of lifestyle changes and the management of chronic conditions. This model is cost-effective and results in less expensive healthcare for patients.
